Individual Coaching Plan Overview
Initial Meeting
We will meet to discuss the following:
Identify a Goal
3. Decide on the goals of our work together after discussing Assessments.
Assessment
What pre-assessment data do you have or intend to collect? How will we be collecting data to make informed decisions together? What type of student work will we be analyzing? We will add these assessments to our Action Plan.
Create an Action Plan
We will collaborate on an Action Plan. This plan will include our shared responsibilities and a timeline for all of our work together. This Action Plan will include opportunities for revision of our goals as needed. If you are requesting PDHs, we can support you in completing the correct forms.
During Coaching Cycle
We will meet a minimum of once a month. We will use this Meeting Log to record our activities during our coaching cycle.The coach will also visit the class(es) once a week. At each meeting, we will engage in several coaching practices to measure student and teacher learning.
Reflection Conversation

We will meet to discuss the following:
- What do you want me to know about you as a teacher and learner?
- We will be using Google Docs to collect information and collaborate. How comfortable do you feel with Google Docs? If you don’t feel comfortable, how can I support you?
- Our norms will include: Respect, Responsibility, Inquiry, Perseverance and Positivity. Do you have any questions or concerns about these norms as they relate to our work together?
Identify a Goal
- What change do you want to see in your classroom? What are your hopes for your students as a result of our work together?
- Choose Area of Focus:
- Planning and Preparation (ex: lesson design, unwrapping standards, incorporating technology)
- Classroom Environment and Management (ex: cooperative learning strategies, classroom management)
- Instruction and Assessment (ex: questioning, assessment literacy, formative assessments/feedback)
- Professional Responsibilities (ex: communication, reflecting on teaching)
